Changes from last version: Dates, clarification of instructions with new procedures for submission
In order to apply for this scholarship, you must be:
- A physician assistant student in good standing in your program as of August 1, 2018, and
- Be starting your final year in your program, and
- Be one of three students recommended for the Scholarship Award by your Program Director.
Applicants must demonstrate the following on their application:
1. Financial Need,
2. Outstanding scholarship,
3. Professional involvement, and/or Community service.
Financial need means that which may impact on the student’s ability to concentrate on academic requirements for successful completion of physician assistant training. Scholarship is determined by GPA. Professional involvement / Community Service can be whatever you believe to pertain: prior or current research, independent and organizational initiated volunteer activities and programs.
The decision matrix is: Financial need 75%, Scholarship 20%, and Professional / Community service 5%.
For 2018, $1,000 will be awarded to one student from each of the six Michigan PA Programs.
Completed Scholarship Applications must be received by email by us by August 30, 2018.
First complete the application, and then simply copy the entire page. Open an email message to Jack at firstname.lastname@example.org, and paste the page in the body of the email. Be sure to complete all required fields before copying.
Scholarship Award recipients will be contacted by September 30, 2018. Formal Award presentation will be made at the Friday Student Challenge Bowl at the Annual Fall Michigan Academy of Physician Assistants CME Conference, The presence of the award recipients (or a stand-in person) at the Award Ceremony and permission for photographs is requested.
Privacy Information: Our Promise to you
We consider the information on the Scholarship Application you provide to be privileged and confidential. Only the Scholarship Application Review Committee will see your information. Particulars will not be shared with other Foundation Board of Directors members or outside parties. It will never be sold, bartered or released to any third parties, including your classmates.
Should we determine that you provided false information on your application, the Foundation will request the Financial Award be returned to us. We will pursue legal avenues should difficulties arise.
The Foundation reserves the right to use your name as a recipient of the Scholarship Award during the Award Presentation, in promotional materials, and to media outlets. Should you be selected to receive an Award, we will ask you to provide a short comment on how the Award was helpful to you in continuing or completing your PA education. We reserve the right to also use these comments in promotional material in fund raising activities.
